Healing the Wounded Child: Utilizing EMDR Therapy with Children and Adolescents with Dissociation

Title: Neurobiology of Trauma & Dissociation and Evaluating Dissociation in Children and Adolescents
Presenter: Frances Waters, DCSW, LMSW, LMFT
Childhood dissociation is often misunderstood or missed with children with complex trauma due to lack of understanding of the animal stress response system and overlapping symptoms that can mask dissociation. Yet, dissociative symptoms can seriously impair the success of treatment.
In this module, a description of the similarities between the Affective Information Processing and structured treatment for dissociation will be explained. Triune Autonomic Nervous System described in Porges’ Polyvagal Theory will describe how dissociation is a last resort of surviving by escaping mentally when a child cannot escape physically from the overwhelming threat of annihilation. A description of the core symptoms of dissociation in children and adolescents will be presented with multiple case examples and drawings. Differential diagnoses of overlapping symptoms of more commonly known diagnoses that mask a dissociative disorder will be described, such as ADHD, psychosis, bipolar disorder. A thorough assessment process that includes posttraumatic and dissociative checklists, interviewing process and questions, and gathering pertinent collateral information will be highlighted.

Title: Identifying and Engaging Dissociative States for Stabilization
Presenter: Frances Waters, DCSW, LMSW, LMFT
This training will focus on the preparation phase for EMDR therapy with specialized designed strategies for children with dissociation. Underlying principles that guide treatment of this population will be addressed for successful stabilization. A mapping of the self-states (ego-states or parts) and their roles or functions will be described for laying the groundwork for effective intervention with EMDR. Numerous examples of effective stabilization techniques with the use of EMDR therapy will be described. Strategies on engaging the child and his/her self-states for developing co-consciousness and collaboration will be illustrated. Struggles and obstacles to attaining stabilization of this population will be described and ways to engage destructive self-states, in particular, will be highlighted. The use of EMDR therapy to encourage safe discharge of negative affect, safe place imagery, and containment exercises will be described. Additionally, specific stabilization techniques that utilize EMDR that encompass self-reflection and expanding the child’s window of tolerance across all parts of the self will also be explained. Engaging the parents/caretakers in supporting their child and participating actively in their child’s treatment will be described. Videotape of a stabilization technique will be shown.

Title: Advanced Preparation Strategies with Dissociative Children
Presenter: Ana M Gomez, MC, LPC
Children that present with dissociative tendencies may become activated during processing and lose dual awareness. Clinicians working with this population need to organize the preparation phase of EMDR therapy in ways that they can assist these children in understanding their dissociative experiences, learn to mindfully measure how present they are and be acquainted with tools that restore present awareness. This module will provide a number of strategies that will aid clinicians in working with various levels of dissociation; mild, moderate and severe. “The dissociation kit for kids” will be thoroughly reviewed providing with it very practical and hands on strategies to help children be fully prepared for the processing phases of EMDR therapy. Moreover, this module will cover ways to help children overcome the phobia of the trauma. Titration, pendulation and “in” and “out” strategies will be addressed.
Clinicians will learn how to assist children and adolescents in creating “the little self healing place” so it can be further utilized during the processing phases. In addition, attachment informed and dyadic resources will be presented to provide younger self-states corrective experiences and meet unmet needs. This module will present strategies to work with more severe dissociation and identify targets with the “inner system” and self-states to create the EMDR treatment plan.
1. Participants will be able to apply strategies from the “dissociation kit for kids” (Gomez) to prepare children with dissociative tendencies. Clinicians will be able to:
- Talk to children about their dissociative experiences.
- Develop ways to measure present awareness with children.
- Use tools to restore dual awareness with children.

2. Participants will be able to apply strategies to Pendulate and titrate amount of trauma to work with children that present with phobia of the trauma and dissociative tendencies.

Title: Dissociation and the Processing Phases of EMDR Therapy with Children
Presenter: Ana M Gomez, MC, LPC
This module will focus on the EMDR processing phases. Strategies coming from recent event protocols as well as the use of EMD and EMDr will be covered as a way of titrating the amount of traumatic material and slow down processing for children that present with complex trauma and dissociative tendencies.
How to work with “parts” and self-states as well as the use of strategies coming from the EMDR-Sandtray protocol to provide the stage and the distance for parts to tell their stories will be demonstrated. During this module, clinicians will learn how to use the “little self healing place” during the processing phases to nurture, support and heal the younger parts. This module will address how to embrace and deal with dissociative responses during the processing of trauma and what to do if dual awareness is lost. The “Interweave Systems Model” will be resented which provides markers and guidelines for selecting interweaves with children presenting with dissociative tendencies.
